Since this has had further correspondence, I
might as well add that I bought it at Clouston & Hall Books (a remainders
shop) in Canberra. It is the 1991 reprint of the 1986 book. I didn't realise it
was so well regarded.

I ran a querie on Biblioquest and
turned up a couple of second-hand copies in the US for around A$70, so I'd
say you got a very fortunate bargain indeed!
