I've had luck with the PIC servo from JRKerr. You'll need to
provide your own amp like the LMD18200 or just buy their completed
board. I'm about to experiment with the LM628 or 9, the one with the quadrateure feedback inputs just so I'm not tied to someone elses firmware. Very low part count interface/driver circuit too. You can also buy something called the Andi-Servo. Its a PC104 card with 2 LM62x chips on it and 3A 50V drivers. We've used those in the past, they work well.
