Filemaker Pro has a system for doing this at several levels of
complexity. I've not really gotten into that, so don't know all the
requirements. I think it would require some software on your server,
which could be a problem if using a commercial service. Check the
choices here:

I use Filemaker Pro for my data. I use a custom field card I designed,
actually two, one for sound one for photos. Then my Filemaker database
duplicates the format of that card for data entry.
I even have my filemaker database in my Palm to cart back into the field
to refer to as necessary. Filemaker makes a Palm version which can be
synched to your main file. It would be possible to set that up to enter
the data directly, but I don't do that. I like having a non-computer
record system in the mix.
I don't bother tying my database into my soundfiles, though it could be
done. I have all my soundfiles out on a hard disk, where with the mac
they are extremely easy to access and play. Filenames are indexed the
same as the database. I do not believe in metadata in the soundfiles or
wasting space with spoken data. Though I usually put some basic stuff
into the mp3's when I make them. In my audio CD backups I go to the
effort to make up a booklet for each CD which has the critical data.
