Did you consider submitting the bird for necropsy?  Most vets are 
happy to investigate such cases, particularly if there is evidence of a chronic 
problem. It is however, preferable to find one who is experienced 
with birds.  A properly performed comestic post mortem also keeps the museum 
happy.
