I tried to do this and have given up. The Mac Disk Utility creates partitions,
with the fourth partition as the first and only partition of the fourth
partition which is an extended partition. You need an extended partition if
you want to have more than four partition, but if you only need four partitons,
you don't need an extended partition.
I ended up using Ubuntu as a virtual machine within Parallels Desktop. That's
where I do my kernel compilations as well. (There is a workaround involving
use case-sensitive filesystems on OSX, but Ubuntu is simple and clean).
Also, within OSX, if it can see a disk it grabs it for itself, so you can't
write the partition table. If you eject it to stop OSX messing around with it,
it's gone for good.
There were still problems fdisk'ing (or gparted) from Ubuntu but it seemed to
be due to unreliable cheap CD cards.
There is also some complexity around UUID disk labels that I never quite got to
grips with Keep it simple and use the right tools for the job - in my case
Parallels + Ubuntu.
