A couple of suggestions:
About 'Category' I would like to also use additional categories--'lecture',
'test', 'travel notes', and a few others. I hope the 'categories' shown are
examples, will not be hard coded and will be user extensible.
I would find a way to include sample rate and bit depth helpful. Would that
go in the [Recording Setup] section?
I would also find it helpful to have a field to input keywords. Perhaps as
a text field, where I could add them, comma separated.
I do not see a place to put a free-form Comment or Description field. Much
of what I would put there could be captured in the database fields you show,
but it would be nice to be able to add a descriptive narrative for about the
place, circumstances, 'artist' statement, or other information. **
