Koss used to make a pair of headphones (Pro 4AA's?) that had oil-filled ear=
 surrounds that were quite impressive at isolation as they moulded themselv=
es to your head. The down side was that they quickly became very uncomforta=
ble to wear!

That may be an unrealistic expectation. The best I know are

UltraPhones, rated at 29 dB isolation.
