Try adding sd_mod to the list of modules you insert. I seem to recall that's 
the one that finally creates the sd* devices.

Also, in order to mount a FAT filesystem, you'll need to insert the fat and 
vfat modules. Those modules may have some other dependencies as well, but a 
Google search should pull up something if you search for "insmod vfat".
