It's probably not set executable, /etc/rc2.d/S99MyApp should be a 
symlink to /etc/init.d/MyApp, and MyApp should be executable

mv /etc/rc2.d/S99MyApp /etc/init.d/MyApp
chown root:root /etc/init.d/MyApp
chmod 755 /etc/init.d/MyApp
ln -s /etc/init.d/MyApp /etc/rc2.d/S99MyApp
