Sounds like the evil moisture monster is hiding in there somewhere.

Subject: Loose capsule head - doubt it. 
Message: 2. 
Subject: moisture in the capsule itself - unscrew it and try another to confirm 
this.

Ive only seen the MX391O version of this capsule which maybe different.

Try another capsule head first, if it still has the problem its most likely 
moisture
somewhere other than the diaphragm and backplate. 
Try to dry it out if you can near a warm object like a TV but not too hot, 
definitely not in an oven. Place the capsule and a desiccant bag together while 
drying.
The desiccant should absorb the moisture providing its also dry to start with.

Woven flouropolymer tape over the capsule head may prevent it but it may also 
trap moisture inside.
