Sennheiser MKH-415T (older version of the 416, but basically the same AFAIK)
in a rycote (just the plain thing no hairs). 
I'd say about 3-5 meters away, but it was dark, so I might be off a bit.
Definantly not closer then 2 and no futher then 5.
