I did the standard tourist cruise five years ago with my wife and one year old. It was full of snorkellers and not set up for birding. That said - I saw a few lifers up close. Lesser created terns, brown boobies amongst others. 

It is a spectacular spot. Someone more knowledgeable may have a better birding option but if you can’t find one then definitely give it a go.  Probably the easiest way to see sea birds near cairns. Ebird has a lot of lists to show what’s been there recently. 

Make sure you pay attention as you approach and leave the cay. I saw a bridled tern on the way back in
